Stock 2000 Silverado LS with 4.8L engine, auto transmission, 198K miles.
Vehicle is properly maintained with only quality parts, fluids and filters.

When the transmission is on 4th gear, the truck will occasionally hesitate and surge. This usually happens when going up low hills.

The hesitation stops if I blip the throttle which drops the transmission to 3rd gear.

Is this indicative of the the transmission gears just being worn or could there be another cause?
